Self-Report

If assigned an Interview or "check-in" by your Officer, you will be requested to complete scheduled Self-Reports. The interview (check-ins) indicate when you are required to check in for the automated interview. Select the Self-Report button on the date required by your Officer or check in periodically as instructed. 
Self-reports are a good time to verify if your contact information or other pertinent information has changed during monitoring
 

To complete an assigned interview, you will complete the following steps:

  • Open the Enrollee App.
  • Select Self Report.
  • Once selected, you will be required to pass either a Voice Verification or Facial Verification.
  • After the verification process is complete, you will complete the automated questions in the interview by selecting an answer and then selecting Next at the bottom of the screen.
  • Upon completing all questions of the interview, you will see a confirmation page that the interview is complete.

Request for written responses:

Avoid using special characters in your written responses to avoid errors.

Interviews older than 26 days cannot be completed. Voice enrollment must be done before an interview can take place.
Can be shared with Enrollees.
